The Public Information Officer is the official custodian of all records which are centrally maintained by the Authority. Some records, including those involving Protected Health Information, are not fulfilled by the Authority and will need to be requested through the Authority's medical service billing provider with the appropriate medical release forms. The Authority will normally produce the requested information within three (3) working days. If extenuating circumstances exist, the information will be produced within seven (7) working days. In accordance with Colorado statute, the Authority may charge for any copies, printouts, or photographs requested, as well as research, retrieval and necessary redaction. The information will be delivered upon payment of any such charges. I understand and agree.
